Dehydration is one of the most common medical conditions EMS providers assess in the field. Although mild dehydration may cause only thirst and fatigue, severe fluid loss can quickly progress to hypovolemic shock, especially in young children, older adults, and patients with prolonged illness or heat exposure.
Early recognition allows EMS clinicians to begin appropriate treatment, prevent complications, and provide receiving facilities with valuable assessment findings.
Common Causes of Dehydration
Dehydration occurs when fluid losses exceed fluid intake, reducing circulating blood volume and affecting tissue perfusion. The body initially compensates by increasing heart rate and constricting blood vessels, but severe fluid loss can eventually lead to hypovolemic shock.
Common causes include:
- Gastroenteritis with vomiting or diarrhea
- Heat exposure and excessive sweating
- Fever
- Prolonged exercise or physical exertion
- Poor oral fluid intake
- Chronic illnesses or medications that increase fluid loss
A thorough assessment helps determine not only whether dehydration is present, but also how severe it has become.
Assessing Dehydration in Adults
Adult patients often report increased thirst, weakness, nausea, or reduced urine output. Patients may also report lightheadedness or dizziness when standing, suggesting decreased circulating blood volume. Asking about recent illness, time spent in hot environments, fluid intake, medication use, and chronic health conditions provides valuable clues during the history.
The physical assessment should focus on identifying evidence of decreased circulating volume. Dry mucous membranes, tachycardia, orthostatic changes, decreased urine output, and declining mental status may all indicate worsening dehydration. Decreased skin turgor may support the assessment, but age-related changes make this finding less reliable in older adults. Providers should monitor blood pressure trends, skin temperature, perfusion, and the patient’s overall presentation rather than relying on any single sign.
Assessing Dehydration in Children
Children can become dehydrated much more quickly than adults because they have higher metabolic demands and smaller fluid reserves. A child who appears only mildly ill can deteriorate rapidly if fluid losses continue.
Parents or caregivers often provide the most helpful history. Ask about vomiting, diarrhea, fever, feeding difficulties, urine output, and changes in activity level. During the physical examination, pay close attention to overall appearance, level of interaction, respiratory effort, and perfusion. Infants and young children may not verbalize thirst, making careful observation especially important.
Signs that suggest pediatric dehydration include:
- Dry mucous membranes
- Decreased tears when crying
- Sunken eyes or a sunken fontanelle in infants
- Delayed capillary refill
- Fewer wet diapers or decreased urine output
- Irritability or lethargy
Because children often maintain their blood pressure until late in shock, abnormal perfusion and changes in mental status may provide earlier warning signs than hypotension.
Patients at Higher Risk for Dehydration
Certain patients are more likely to develop severe dehydration and may deteriorate more quickly than healthy adults. Higher-risk groups include:
- Infants and young children
- Older adults
- Patients with chronic kidney disease
- Patients taking diuretics
- Individuals working or exercising in extreme heat
- Patients with prolonged vomiting or diarrhea
Recognizing these risk factors during the patient history helps EMS providers anticipate more serious illness and prioritize ongoing assessment.
Recognizing the Severity of Dehydration
Dehydration exists on a spectrum. Patients with mild dehydration may simply report thirst and demonstrate minimal physical findings. As fluid losses increase, tachycardia, delayed capillary refill, decreased urine output, weakness, and dizziness become more apparent. Severe dehydration may present with altered mental status, poor peripheral perfusion, hypotension, and signs of hypovolemic shock.
Rather than relying on a single symptom, EMS providers should evaluate the patient’s overall presentation, trending vital signs, history, and response to treatment. Recognizing progression is often more valuable than identifying one isolated assessment finding.
EMS Treatment Priorities
Treatment begins with a thorough primary assessment and continuous reassessment throughout patient care. Providers should identify life threats, determine the severity of dehydration, and manage any underlying illness contributing to fluid loss.
EMS priorities include:
- Evaluate the patient’s surroundings for clues, such as excessive heat exposure, limited access to fluids, strenuous physical activity, or evidence of prolonged illness.
- Complete the primary assessment and address immediate life threats.
- Monitor vital signs, perfusion, urine output when known, and mental status throughout care.
- Check blood glucose when indicated.
- Provide an appropriate oral rehydration solution or intravenous fluids according to the patient’s condition, provider level, and local protocols.
- Reassess frequently for changes in vital signs, mental status, and perfusion.
- Transport patients whose symptoms, risk factors, persistent fluid losses, or signs of poor perfusion require further evaluation.
Patients with significant dehydration, persistent vomiting, altered mental status, or evidence of shock require prompt transport and ongoing monitoring.
Putting Assessment into Practice
Dehydration is a common call, but it still requires thoughtful clinical judgment. The signs may be subtle, the presentation may vary by age, and the patient’s condition may change during your care. A systematic assessment helps you recognize when a routine complaint is becoming something more serious, and respond accordingly.
With experience, recognizing those changes becomes part of seeing the whole patient rather than focusing on a single sign or vital sign. Stay attentive to what you see, what the history tells you, and how the patient responds over time. In the field, those details can make the difference between simply identifying dehydration and recognizing a patient who is beginning to deteriorate.
